In consideration of the benefits and opportunities afforded to me by participation in the above named event/activity, I state as follows:
I agree to abide by all rules and regulations pertaining to this event and all rules as set forth by the Maids of Athena, Sons of Pericles, Daughters of Penelope and Order of AHEPA regarding their events and activities. I accept full responsibility and liability for any charges, damages or claims arising from any damages or injuries that I may cause by my actions. I authorize any adult representative of the Maids of Athena, Sons of Pericles, Daughters of Penelope or Order of AHEPA to obtain emergency medical treatment for me in the event of my incapacity or unconsciousness while attending or participating in this event, as deemed necessary and I agree to pay for any expense incurred for said treatment. I understand that under the law of the state where this event shall take place, if I am riding in a private passenger automobile which is involved in an accident, I may be primarily covered for bodily injury under the private passenger automobile insurance policy on the car I am riding in, and that my own automobile insurance will secondarily be responsible for payment of any medical bills or other damages incurred. If my policy has a deductible clause relative to the personal injury protection, I assume the deductible amount.
I fully understand and agree that the Maids of Athena, Sons of Pericles, Daughters of Penelope, Order of AHEPA, their officers, agents, employees, assistants, members or respective heirs and successors, shall NOT be responsible in any way for any injuries or damages that I may incur in connection with my participation, and attendance at this event or any ride going to or from any part of this event.
I hereby release the Maids of Athena, Sons of Pericles, Daughters of Penelope, Order of AHEPA and all their officers, agents, employees, assistants, members and their respective heirs and successors, from any liability whatsoever for any accident or injury that may occur to me in connection with my participation in the event described above.
I also fully release the Maids of Athena, Sons of Pericles, Daughters of Penelope, Order of AHEPA and all their officers, agents, employees, assistants, members and their respective heirs and successors, from any liability whatsoever in connection with any actions, damages, or injuries that I undertake or commit at any time during or associated with this event. I fully understand that I am responsible for payment of all charges for all damages or injuries that I cause.
